Attempts to staunch a Legionnaire’s disease outbreak at two Pittsburgh Veterans Affairs (VA) hospitals were mystifyingly unsuccessful according to officials’ testimony to Congress, but internal VA emails showed some of those officials were aware that efforts to cleanse the hospitals’ water systems were inadequate – and led to at least six deaths and sickened sixteen other patients.
